Social Worker - Discharge to Assess (D2A) - Birmingham

Hours

Monday to Thursday 8:30am–5pm, Friday 8:30am–4:30pm
Hybrid working — 1 day in office per week

The role of Social Worker:

Deliver person-centred practices enabling citizens to exercise choice, control, and independence
Undertake assessments, support planning, and 3 Conversations model interventions in accordance with the Care Act 2014
Work within the Discharge to Assess Community Social Work Team supporting citizens post-hospital discharge
Handle complex Homeless Pathway cases covering substance misuse, safeguarding, and multidisciplinary settings
Apply extensive knowledge of the Care Act 2014, Safeguarding, Mental Health Act, and Mental Capacity Act 2005
MDT working with partner agencies

The ideal Social Worker:

Recognised professional Social Work qualification (MA/BA/BSc, Diploma in Social Work, or CQSW)
Social Work England registration
Minimum 2 years post-qualifying permanent experience

Birmingham City Council's Discharge to Assess Service, supporting citizens who have left hospital with short-term services requiring longer-term support planning.
